Rehabilitative Therapeutic Intervention on Dropped Head Sign in Parkinson¡¯s Disease: A Case Report

A 67-year-old woman with Parkinson¡¯s disease (PD) visited our outpatient clinic displaying the dropped head sign (DHS), which is a term of severe neck flexion posture. However, the occurrence of DHS in PD is rare. Surface-electromyography revealed that neck extension did not significantly increase the activity of the neck flexor muscles (NF), while that of the neck extensor muscles (NE) showed minor increase. Thus, the main cause of DHS was unlikely to be dystonia of NF but a weakness of NE instead. Comprehensive rehabilitation was performed, including strengthening exercises and electrical stimulation of NE. We also prescribed postural education of neck extension and a spinal orthosis. After rehabilitation, the patient could lift her head more easily and the pain in her neck was reduced. This case highlights the fact that understanding the cause of DHS in PD is important in establishing the appropriate management plan for the condition.
